A pack-a-day habit can run a smoker roughly $14,000 a year, on top of well-documented damage to health, yet quitting remains one of the hardest behavioral changes many people ever attempt. New research from Adelaide University adds a practical, low-cost option to the toolkit: exercise, the study finds, meaningfully improves the odds that a quit attempt will succeed.
The systematic review and meta-analysis pooled data from 59 randomized controlled trials involving more than 9,000 participants, examining how both single bouts of exercise and sustained exercise programs affect smoking cessation, cravings, withdrawal symptoms, and mood. The results, published in the Journal of Sport and Health Science, found that people enrolled in exercise programs were 15 percent more likely to achieve continuous abstinence and 21 percent more likely to report not smoking over a seven-day period, compared with control groups who didn’t exercise.
Exercise also reduced how much people smoked in the first place: participants in exercise programs cut their cigarette consumption by roughly two cigarettes a day. And the benefits weren’t limited to long-term programs. A single session of exercise immediately blunted cigarette cravings for up to 30 minutes afterward, a window that researchers say could make a real difference during the hardest moments of a quit attempt.
Lead researcher Dr. Ben Singh said the findings hand smokers a practical, low-cost tool to support their efforts to quit. Quitting smoking, he noted, is one of the best things a person can do for their health, but it is also one of the hardest, and because current approaches don’t work for everyone, more low-cost strategies that fit into daily life are badly needed. Something as simple as regular exercise, he said, can make a meaningful difference by helping people manage cravings, smoke less, and improve their overall chances of quitting for good.
The stakes are considerable. Tobacco smoking remains the leading preventable cause of premature illness and death worldwide, responsible for an estimated 7 million deaths annually, including roughly 1.6 million among non-smokers exposed to secondhand smoke. At the same time, the nicotine landscape keeps shifting: e-cigarette use has climbed past 100 million people globally, even as traditional smoking rates have declined over the past two decades. Demand for e-cigarettes and heated-tobacco products has risen in particular among younger users, and today roughly 80 percent of the world’s 1.3 billion tobacco users live in low- and middle-income countries, even as vaping use has climbed sharply across many wealthier OECD nations.
Senior researcher Professor Carol Maher said the findings point to exercise as a strategic tool smokers can deploy against cravings in the moment, not just as a long-term lifestyle change. Quitting, she said, does not have to rest on willpower alone. Cravings can be tough to manage, but they typically pass, and the review found that even a single bout of exercise can reduce cravings for up to 30 minutes, potentially helping people get through some of the hardest stretches of a quit attempt. She was careful to frame exercise as a complement rather than a substitute for established treatment: it should not replace evidence-based supports such as counseling and smoking-cessation medication, she said, but it may serve as a practical, low-cost strategy people can use alongside them.
The research team’s next steps involve testing how exercise interventions might be woven into real-world quit programs, whether delivered through digital platforms, community initiatives, or clinical services, and examining whether similar benefits extend to people trying to quit vaping, an area where the evidence base remains thin.
For public health systems weighing how to respond to a nicotine landscape that now spans combustible cigarettes, e-cigarettes, and heated-tobacco products, the appeal of exercise as an adjunct treatment is its accessibility: unlike prescription cessation aids, it costs little or nothing and can be layered onto whatever support a person is already using. Whether that promise translates into durable, population-level results will depend on how well the strategy performs once it moves out of controlled trials and into everyday quit attempts.
